Public Health Nursing

Public Health Nurses are Registered Nurses who have additional training and certification to work in the community and the field of Public Health. Our goal is to meet preventive health needs of families and individuals, and promote positive health behavior. Our nurses offer in-home care to new parents, children, and the elderly.

  • Parenting
  • Post-Partum Depression screening

Family & Mental Health

PH Nurses can help:

  • Provide mental health information
  • Assist with finding a Doctor
  • Provide disease information
  • Exercise, health & wellness education

Elder Care

PH Nurses can offer:

  • Exercise, health & wellness education
  • Medication assessment and information
  • Assistance with medical equipment needs
  • Medical Information
  • Referrals for food assistance and medical insurance
  • Assistance with personal needs

Getting Started

If you or someone you know could benefit from Public Health Nursing assistance, start by submitting a Public Health Nursing Referral Form to our office. Once received, our nurses contact the referred individual to offer services.

If PH Nursing services are accepted, a nurse will make an in-home visit and perform a comprehensive needs assessment of individuals and the family as a whole. The needs assessment typically takes about one hour, though can take longer.

The needs assessment allows the nurse to develop a specific care plan which may include:

  • Referrals for medical care and related resources
  • Nutrition assessment and referrals
  • Service coordination
  • Health counseling
  • Wellness program referrals
  • Breastfeeding and postpartum depression referrals and support
  • Medically fragile infant home visitation
  • New baby care and growth and development information
